LTO Certificate of Authentication

(For Acquisition of Japanese Driver’s License)

  1. Original LTO Certificate authenticated by the Department of Foreign of Foreign Affairs plus one (1) copy.

  2. One (1) copy of the Philippine Driver’s License.

  3. One (1) copy of the Official Receipt (OR) of the Philippine Driver’s License.

  4. One (1) copy each of the data, visa and amendment (if any) pages valid passport

  5. Return mail (Letter Pack 500 envelope – available at the Post Office)
