Two (2) photocopies of the data and visa pages of the applicant/s valid passport:
For joint SPA, ASGC and other Affidavits, if the other signatory is Japanese or of other nationality, two (2) photocopies of the data page of his/her passport.
If applicant/s could not appear in person, the document/s should be:
Notarized by a Japanese notary public (koshonin yakuba);
Authenticated by the Legal Affairs Bureau (homukyoku);
Authenticated by Ministry of Foreign Affairs of Japan in Osaka (Gaimusho ), Certification Section (shomei-han) (tel. no. 06-6941-7000);
Return mail (Letter Pack 500 – available at the Post Office).
Additional requirements for Affidavit of Support/Guarantee and Consent (ASGC) intended for Japanese or other foreign passport-holder children of a Filipino parent travelling to the Philippines unaccompanied by either parent, for purposes of securing Waiver of Exclusion Ground (WEG):
Two (2) photocopies of the child’s passport (data page)
Two (2) photocopies of the accompanying adult’s passport (data page)
Authentication of SPA, ASGC, Affidavits and other documents signed by Japanese citizens or other nationalities.
Please follow the following procedures. Have the document/s:
Notarized by a Japanese notary public (koshonin yakuba)
Authenticated by the Legal Affairs Bureau (homukyoku)
Authenticated by the Ministry of Foreign Affairs of Japan (Gaimusho) in Osaka, Certification Section (shomei-han), telephone number 06-6941-4700
Bring or send the authenticated documents to the Philippine Consulate for authentication.
