Personal appearance. Applicant/s must be at least 18 years old.
Duly accomplished application forms, available at the Consulate
Three (3) passport size photos each showing left side view, front view and right side view.
Original and two (2) photocopies of at least two of the following documents as proof of being a natural-born Filipino citizen:
Authenticated Philippine birth certificate
Philippine passport
If married in the Philippines, original marriage certificate from NSO authenticated by the DFA
If married in Japan or other countries, certified true copy of the report of marriage from the respective Philippine Embassy or Consulate.
Other official document/s indicating that the applicant is a former natural born-Filipino citizen.
Photocopy of foreign passport;
Photocopy of the Certificate of Naturalization, if not available, sworn affidavit stating how foreign citizenship was acquired.
