Personal appearance. Applicant/s must be at least 18 years old.
Duly accomplished application form, available at the Consulate.
Original and two (2) photocopies of the Notice of Approval of Citizenship from the Government of Japan.
Original and two (2) photocopies of the Japanese passport issued to the applicant.
In the absence of Notice of Approval of Citizenship and Japanese passport, the applicant must bring his/her original Kosekitohon (family register) and Juminhyo (residence certificate)
Two (2) signed Oath of Renunciation, form available at the consulate.
Three (3) passport size colored photos.
