REPORT OF BIRTH

Filipino children born outside of the Philippines must secure a report of birth (RB) at the country where the child was born. It is a requirement when the child applies for a new passport.
 
Fee: 4,850 yen, which includes the translation fee of the Japanese birth certificate.

Affidavit of delayed registration: 2,425 yen, for a total amount of 7,275 yen.

When the parents are married (legitimate children), the requirements below should be grouped in four (4) sets:

  1. Completed report of birth form (FA Form 40). Download form here   It is important that one of the parents (or the physician/nurse) sign at the blank line for this purpose.

  2. Original copy of the child’s birth certificate (shussei todoke), issued by the city hall (shiyakusho). Please make 4 photocopies.

  3. Four (4) photocopies of parents’ NSO-issued marriage contract if married in the Philippines, or report of marriage if married in Japan.

  4. Four (4) photocopies of the Filipino parents’ NSO-issued birth certificate.

  5. Four (4) photocopies of the parents’ passport pages (the data page, visa page, and last page). If a parent is a Japanese or another foreigner, three photocopies of his/her ID, such as passport or driver’s license.

  6. ADDITIONAL REQUIREMENT: If the report will be filed 30 days or more after the birth of the child, please fill up one copy of the affidavit of delayed registration of birth. Download form here 

  7. ADDITIONAL REQUIREMENT: If the child was born when the parents were not yet married, and the parents are now married, please submit an affidavit of legitimation. Download form here

When the parents are not married (illegitimate children), the requirements are basically the same, but there is no need to submit the documents of the biological father. Also, in the RB application form, the column for the father is left blank.
 
However, if the biological father recognizes the child as his, the applicant should submit the documents of the father, along with an Affidavit of Admission of Paternity. Download form here  Moreover, if the father agrees to have the child use his family name, the applicant should add an Affidavit to Use the Surname of the Father. Download form here   In these cases, the column for the father on the RB form can be filled up.
 
REMINDER: Please apply personally at the Consulate and in order to mail back the report of birth to your address, please include a large (A4 size) self-addressed envelope with 900 yen stamps, or an ExPack 500 envelope.
