Requirements for Non-Quota
Immigrant Visa (13-A)
Applicant must be
married to a Filipino citizen or unmarried children under twenty-one (21) years
of age of a Filipino citizen, if accompanying or following to join such
citizen. Applicant must submit the following documents:
- Application form for immigrant visa
(FA
Form No. 3) duly accomplished in triplicate. (Note: Please print both pages on one A4 size paper (front and
back)).
- Certified true copy of applicant’s Birth
Certificate / Family Register.
- Authenticated Birth Certificate of applicant’s
Filipino spouse and authenticated Marriage Contract.
- Affidavit stating that applicant intends to reside
permanently in the Philippines
to be notarized in the Philippine Consulate.
- Affidavit of Financial Support or proof of means of
support to be notarized by the Philippine Consulate.
- Six (6) photographs (5cm x 5cm) with white plain
background, signed on the front left side.
- Passport valid for at least one year from the date
of application.
- Police Clearance Certificate issued by the national
police authorities in the applicant’s country of origin or legal
residence. The Certificate must be valid for at least six (6) months prior
to date of filing of application.
- Medical Examination, Mental, and AIDS Clearance
(FA
Form No. 11) and an X-Ray
film in envelope. Medical Report must be valid for at least six (6) months
prior to date of filing of application.
- Three (3) photocopies of the data and visa page of
the passport of applicant’s Filipino spouse.
Reminders:

Personal appearance is required.

All documents issued by the Japanese government
agencies should first be authenticated by the Ministry of Foreign Affairs of
Japan/Gaimusho before submitting to the Consulate for
authentication.

All documents issued by private agencies should
first be notarized by local notary public before submitting to the Consulate
for authentication.
·
English translation is required for documents
issued in foreign language.
·
Please submit above documents in triplicate,
arranged in the following order/sequence:
First set - all original documents
Second set - original
(FA Form No. 3) and copies of all other documents
Third set - same
as second set
